Physical Certificate Delivery Instructions for GISBeX Clearing Corp

If you have stock or bond certificates that you would like to deposit into your Membersite customer account, follow these 4 easy steps below:

Please note: securities with restricted legends (usually indicated by a big red inked stamp on the front or back of the certificate) are not currently being accepted.

1. Properly endorse the stock certificate and/or stock power, and get it medallion-stamp signature guaranteed. Please write your moneylinebrokers.com account number on the front upper right hand corner of each certificate.

2. Fill out a 'Third Party Release' form assigning the stock certificate to GISBeX Clearing Corp , and have your signature notarized by a Legal Notary.

3. Send all stock certificates and documents together, by overnight mail only, to:

4. Notify your Membersite customer service representative by using your internal email, and advise of the mailing/forwarding of the physical certificates, including the following information for each certificate:

1. Certificate #
2. Certificate Issuer
3. The number of shares
4. The name the certificate is registered in

Following these simple instructions will help insure that your stock is credited to your Membersite account as quickly as possible, thank you.

For proper credit to your customer account, only remit cash and securities to GISBeX Clearing Corp , via authorized GISBeX depository bank/brokerage accounts held in the name of GISBeX Clearing Corp
